 In the procedure, a small hole was made in the areola or nipple and the areola diameter was reduced by applying thermal energy on the rear side of the areola. 

It is the case when gynecomastia causes the widened areola. If the fat and glandular line are properly removed through gynecomastia surgery, the areola is reduced much.
